The defect is in the parking mechanism itself: with the shift lever placed in Park, the park pawl does not always abut or engage the park gear, so the transmission may not actually lock the driveline. NHTSA's stated consequence is that the driver may believe the van is safely in Park when it is not — leading to the Aerostar jumping out of gear on its own, rolling away when left parked on a grade, or moving unexpectedly at startup.
